Withdrawal papers

Unofficial transcript for previous school if
in 12th grade

Two separate utility bills listed in Parent's
or Legal Guardian's name

At least one of the following:
1. Drivers license showing the reported address
2. Notarized statement of residency from landlord

SCHOOL FOR ANY
REASON AND WANT TO ENROLL WE MUST HAVE:

All incoming 9th graders who are socially
promoted from Central, Austin or Weis, must have a letter from the school
principal.
FOR ALL STUDENTS AT BALL HIGH SCHOOL:
If you live with anyone other than your parents
or legal guardian, you must have a Resident Form from Mr. Don Roy. His
office is located at the Support Center, 3904 Avenue T,

Ball High Attendance Policy
A child between the ages of 6 and
17, depending on when the child's birthday falls, is required to attend school
each day during the instructional year unless otherwise exempted by law.
School employees are compelled by law to investigate and report any violations
of the state compulsory attendance law. These attendance requirements also
apply to any District-required tutorial sessions.
Note: Perfect
Attendance honors will be awarded only to students with no absences.
